- Coordinating care with your referring or primary care physician
- Sending follow-up care summaries or consult letters
- Administrative tasks such as appointment booking or requisitions

We require your express or implied consent to collect and share your information, except where otherwise required by law. You have the right to:

- Correct inaccurate information
- Withdraw consent for the sharing of your information (with some exceptions under HIA)
Protecting Your Information
We take every reasonable step to safeguard your health information, including:
- Using encrypted, secure platforms for communication and video appointments
- Limiting access to your information to authorized clinic personnel
- Storing records securely in a Canadian-compliant EMR system. In some cases, for patients who choose to self-refer themselves, we collect limited personal health information through Microsoft Forms. These forms are securely hosted within our Canadian-based Microsoft 365 Business environment and are accessed only by authorized clinic staff in accordance with Alberta’s Health Information Act (HIA).
